- You provide nursing care: you perform both nursing and medical procedures entrusted to you by the doctor in accordance with the legal provisions.
- You provide healthcare to the residents as laid down in the Basic Law (continuity of care, contributions to prevention and reintegration).
- You observe the residents to contribute to making a correct diagnosis and this in collaboration with the psychomedical care team.
- You contribute to the treatment of the residents with a view to stabilize and/or improve the psychological and physical health. This can be through providing psycho-education and offering guidance with regard to personal and house hygiene.
- You work out an individual treatment plan in collaboration with the psychomedical care team.
- You offer a client-oriented service through individual conversations and individual guidance and you build a good relationship with the other actors in the transition house.
- You represent the Health Service: you analyse data related to the health of residents and promote health care in the house.
- You maintain good relationships with external partners that cooperate with the transition house.
- You provide immediate support and intervention during mental health crises, including de-escalation techniques.
- You offer both individual and group therapy sessions to address residents' mental health concerns and promote emotional well-being.
- You educate residents about their mental health conditions, treatment options, and coping strategies.
- You maintain accurate and up-to-date medical records, progress notes, and incident reports.
- Registered Nurse (RN) or Advanced Practice Registered Nurse (APRN) with a specialization in psychiatric nursing.
- Minimum 8 years of relevant experience in social work and related fields
- Knowledge of mental health disorders, including PTSD, depression, and anxiety.
- Strong communication and interpersonal skills.
- Ability to work as part of a multidisciplinary team.
- Empathy, compassion, and a commitment to providing trauma-informed care.
- Familiarity with crisis intervention techniques and de-escalation strategies.
- French and English, Dutch is a plus.
